We would like to ask you about your practice or opinions about delivering bad news in ALS(amyotrophic lateral sclerosis). Our purpose of this study are:

  1. To present the current status of delivering  bad news in ALS to elucidate  difficulties and the tasks.
  2. To propose a future direction to improve delivering  bad news in ALS.
  3. To present the overview on how doctors working in Asia or Oceania deliver bad news in ALS. In addition, we attempt to compare their way to deliver and  difficulties between the countries.
  • PARTICIPATION:Your participation in this survey is voluntary. You may refuse to take part in the research the survey at any time without penalty. It takes about 7minutes to answer.
  • BENEFITS:You will receive no direct benefits from participating in this research study. However, once all the questions have been answered, you may read the EFNS guidelines for delivering the bad news, to help in your practice.
  • RISKS:The possible risks or discomforts of the study are minimal. If you feel bad answering this survey questions, kindly contact us ,please.
  • CONFIDENTIALITY:Your survey answers will be sent to a link at SurveyMonkey.com where data will be stored in a password protected electronic format. Survey Monkey does not collect identifying information such as your name, email address, or IP address. At the end of the survey you will be asked your contact information. If you choose to provide the information, your survey responses may no longer be anonymous to the researcher. However, no names or identifying information would be included in any publications or presentations.
